Job summary

Aultman Health Foundation is seeking a nurse leader to provide clinical direction for patient care, conduct patient and family teaching, and collaborate with the interdisciplinary team to uphold nursing standards.

The role requires an OH RN license, BSN preferred, BLS certification, and 2+ years of relevant nursing experience. Strong communication and computer skills, plus a valid OH driver's license, are essential. On-site position with family-centered care focus and HIPAA compliance.

Qualifications

• Graduate of an approved and accredited school of nursing; BSN preferred.
• Current State of OH RN license.
• Two years recent nursing experience in acute, long-term care or homecare/hospice, with hospice/homecare preferred.
• Must be able to work independently, have strong patient teaching skills and strong written and verbal communication skills.
• Current BLS certification.
• Must have general computer skills.
• Valid OH Driver's License and daily access to reliable and insured vehicle.

Responsibilities

• Assesses patient and family needs and develops an individualized plan of care.
• Documents problems, nursing assessments, goals, care provided and outcomes.
• Oversees daily assignments and schedules; takes new orders and communicates them to staff.
• Coordinates care with interdisciplinary team; supports patient/family teaching and cultural/spiritual needs.
• Ensures infection control and HIPAA confidentiality; adheres to agency policies.
