Job summary

Qualifications

• RN licensed to practice in the state.
• Graduate of NLN-accredited school of nursing.
• Two years’ experience, preferably in public health or home care.

Responsibilities

• Completes initial patient/family assessment for home care needs.
• Initiates and revises care plans based on patient status.
• Administers medications and treatments as prescribed by physician.
• Counsels patient and family on nursing and related needs.
• Identifies discharge planning needs and coordinates care.
• Acts as Case Manager for assigned caseload.
• Prepares clinical notes and communicates with physicians and care team.
• Participates in on-call duties and ensures equipment/ supplies are available.
