Qualifications

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates or revises plan of care based on patient status.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on nursing and related needs.
• Provides health care instructions per assessment and care plan.
• Coordinates patient care as Case Manager when assigned.
