Job summary

Southcoast Health System, Inc. is seeking an Advanced Practice Practitioner to join the Health at Home - Supportive Care Hospice - Palliative Consulting Team. This full-time, day-shift role offers a structured environment with no weekends, holidays, or on-call.

Responsibilities include collaborative care delivery, patient and family education, and advanced nursing practice in hospice settings. Requirements include an MSN for nursing, NP certification, MA and RI RN licenses, and at least 5 years

Qualifications

• MSN required for NP role.
• Current NP certification is required.
• RN license in Massachusetts and Rhode Island is required.
• Minimum 5 years of nursing experience; oncology, hospice, geriatrics or ICU preferred.
• Minimum one-year NP experience.
• Certification in Hospice and Palliative Care required; CPR certification.

Responsibilities

• Collaborate with physician and palliative care team to deliver hospice care and related services.
• Perform bedside evaluations, assist with diagnosis, treatment, and management of health conditions, and educate families and caregivers.

We are searching for a talented Advanced Practice Practitioner to join our Health at Home - Supportive Care Hospice - Palliative Consulting Team.

Hours: Full-time - 40 hours8A - 4:30P Monday - FridayShift: Day - no weekend, holiday or on-call

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Responsibilities

• Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families.
• Perform a wide variety of duties including but not limited to bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc, and serving as a nursing resource for education and consultation to members of the interdisciplinary team.

Qualifications

• Requires a Masters of Science Degree in Nursing.
• Certification from a national accrediting body to practice as a Nurse Practitioner is required.
• Current RN license in the Commonwealth of Massachusetts & Rhode Island is required.
• Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.
• A minimum of one-year Nurse Practitioner experience.
• Certification in Hospice and Palliative Care required. Must possess current CPR certification
• Certification from a national accrediting body to practice as a Nurse Practitioner is required.
• Current RN license in the Commonwealth of Massachusetts is required.
• Certification in Hospice and Palliative Care required.
• Must possess current CPR certification.

Pay Range

USD $59.41 - USD $71.45 /Hr.
