Must meet Medicare Conditions of Participation for Home Health Aide training program and competency. If currently a licensed CNA, GPHH can provide Home Health Certification
Have a sympathetic attitude toward the care of the sick and elderly.
Ability to carry out directions, read and write.
Maturity and ability to deal effectively with the demands of the job.
This position reports to Home Health Director and is supervised by RN's, PT's, OT's, SLP's
Understands and adheres to established Agency policies and procedures.
Performs personal care, bath, and hands-on care as assigned.
Completes appropriate visit records in a timely manner as per Agency policy.
Reports changes in the patient's condition and needs to the RN.
Performs household services essential to health care in the home as assigned.
Ambulates and exercises the patient as assigned.
Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.
Assists with medications that are ordinarily self-administered as assigned.
Attends in-service and continuing education programs as scheduled and necessary.
Attends patient care conferences as scheduled.
Home care and home health background.
Experience working in a home healthcare environment.
Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.
Knowledge of the local healthcare market and community outreach strategies.
Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky
Knowledge of office management systems and procedures.
Clean driver's license with a clean driving record (Preferred).
