Job summary

Granite Peak Home Health is seeking compassionate Home Health Aides to join our growing team. You will perform personal care, light household tasks, and document visits, while adhering to agency policies.

We welcome candidates who meet Medicare COP for HHA training, with CNA licensure or eligibility. Strong reading and writing skills, reliability, and empathy are essential for success in this hybrid role.

Qualifications

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
• Have a sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.

Responsibilities

• Understands and adheres to established Agency policies and procedures.
• Performs personal care, bath, and hands‑on care as assigned.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient’s condition and needs to the RN.
• Performs household services essential to health care in the home as assigned.
• Ambulates and exercises the patient as assigned.
• Performs simple procedures as an extension of the therapy or nursing services, e.g., ROM exercises as assigned.
• Assists with medications that are ordinarily self‑administered as assigned.
• Attends in‑service and continuing education programs as scheduled.
• Attends patient care conferences as scheduled.

• Assists with medications that are ordinarily self‑administered as assigned.
• Attends in‑service and continuing education programs as scheduled and necessary.
• Attends patient care conferences as scheduled.

Preferred Qualifications

• Home care and home health background.
• Experience working in a home healthcare environment.
• Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.
• Knowledge of the local healthcare market and community outreach strategies.
• Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky
• Knowledge of office management systems and procedures.
• Clean driver’s license with a clean driving record (Preferred).
