Job summary

Granite Peak Home Health is hiring for a Home Health Aide role in Montana. The position emphasizes compassionate patient care, adherence to agency policies, and timely documentation of visits. Training and certification opportunities are available for CNA holders.

The role supports personal care, mobility assistance, and coordination with RNs, with a focus on home-based health services and ongoing professional development.

Qualifications

• Must meet Medicare COP for Home Health Aide training and competency.
• If licensed CNA, Granite Peak Home Health can provide Home Health Certification.
• Sympathetic attitude toward care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to meet job demands.

Responsibilities

• Performs personal care, bath and hands-on care as assigned.
• Completes visit records in a timely manner per agency policy.
• Reports changes in patient condition to the RN.
• Performs household services essential to health care in the home as assigned.
• Assists with ambulation and exercises as assigned.
• Performs ROM exercises as assigned as an extension of therapy or nursing services.
• Assists with medications ordinarily self-administered as assigned.
• Attends in-service and continuing education programs as scheduled.
• Attends patient care conferences as scheduled.

Qualifications:

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency. If currently a licensed CNA, GPHH can provide Home Health Certification
• Have a sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.

Principal Responsibilities:

• Understands and adheres to established Agency policies and procedures.
• Performs personal care, bath, and hands‑on care as assigned.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient’s condition and needs to the RN.
• Performs household services essential to health care in the home as assigned.
• Ambulates and exercises the patient as assigned.
• Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.
• Assists with medications that are ordinarily self‑administered as assigned.
• Attends in‑service and continuing education programs as scheduled and necessary.
• Attends patient care conferences as scheduled.

Preferred Qualifications

• Home care and home health background.
• Experience working in a home healthcare environment.
• Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.
• Knowledge of the local healthcare market and community outreach strategies.
• Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky
• Knowledge of office management systems and procedures.
• Clean driver’s license with a clean driving record (Preferred).
