Job summary

Granite Peak Home Health in Great Falls, MT is seeking a compassionate Home Health Aide to support patients in their homes. You will perform personal care, monitor condition, and collaborate with RNs and therapists to promote safety and well‑being.

Requirements include Medicare COP eligibility and a caregiver mindset. Training and ongoing education are provided, with a flexible schedule and supportive team in a locally trusted agency.

Qualifications

• Must meet Medicare COP for Home Health Aide training and competency.
• Current CNA license can be leveraged for Home Health Certification.

Responsibilities

• Performs personal care and hands-on care as assigned.
• Adheres to agency policies and procedures.
• Completes visit records promptly per policy.
• Reports patient changes to the RN.
• Provides household services essential to in-home health care.
• Assists with ambulation and prescribed exercises.
• Assists with self-administered medications as directed.
• Attends trainings and patient care conferences as scheduled.

Qualifications

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency. If currently a licensed CNA, GPHH can provide Home Health Certification
• Have a sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.

Principal Responsibilities

This position reports to Home Health Director and is supervised by RN's, PT's, OT's, SLP's

• Understands and adheres to established Agency policies and procedures.
• Performs personal care, bath, and hands-on care as assigned.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient's condition and needs to the RN.
• Performs household services essential to health care in the home as assigned.
• Ambulates and exercises the patient as assigned.
• Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.
• Assists with medications that are ordinarily self-administered as assigned.
• Attends in-service and continuing education programs as scheduled and necessary.
• Attends patient care conferences as scheduled.

Preferred Qualifications

• Home care and home health background.
• Experience working in a home healthcare environment.
• Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.
• Knowledge of the local healthcare market and community outreach strategies.
• Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky
• Knowledge of office management systems and procedures.
• Clean driver's license with a clean driving record (Preferred).
