Job summary

BAYADA Home Health Care is seeking a full-time Licensed Practical Nurse to perform home health visits for adult and geriatric clients in Quincy, Stoughton, Randolph, Holbrook, Avon and surrounding towns.

One year of LPN clinical experience is required. Massachusetts license is necessary to join our compassionate team delivering high-quality in-home care with a focus on comfort and independence.

Qualifications

• Current LPN license (Massachusetts) is required.
• Graduation from an accredited nursing program.
• One year of clinical LPN experience required.

Responsibilities

• Follow designated care plans per patient needs.
• Make home visits to clients in geographic territories.
• Administer medications, wound care, and procedures as assigned.
• Monitor client conditions and report changes to Clinical Manager.
• Document doctors' orders and observations accurately.
• Conduct client assessments and coordinate case management.

BAYADAHome Health has an immediate opening for a full-timeLPN, Licensed Practical Nurse to perform home health visits, providing exceptional home health care to adult and geriatric clients in Quincy, Stoughton, Randolph, Holbrook, Avon and surrounding towns. One year prior clinical experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse Responsibilities:

• Follow a designated care plan in accordance with patient’s needs
• Makehome visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; reporting changes to Clinical Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet.

Home Health LPN Visits Qualifications:

• A current license as an LPN in Massachusetts.
• Graduation from an accredited and approved nursing program.
