Job summary

BAYADA Home Health Care is seeking a full-time Licensed Practical Nurse to perform home health visits for adult and geriatric clients in Fitchburg, Leominster, Harvard, Westford and surrounding towns. MA LPN license required; prior home health experience preferred.

Compensation includes a base pay range of $45.00–$50.00 per hour, plus quarterly bonus opportunities, and a comprehensive benefits plan. Immediate consideration via call or text to Susan at 339-235-4363.

Qualifications

• Current Massachusetts LPN license is required.
• Prior clinical experience as an LPN is required.
• Experience in home health settings is preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications and provide wound care and treatments.
• Monitor clients' conditions and report changes to managers.
• Follow up on doctors' orders and document them accurately.
• Perform client assessments as needed and coordinate care.
• Coordinate case management and inter-team communication.
• Document observations and interventions in EMR on a tablet.

BAYADA Home Health has an immediate opening for a full-time Licensed Practical Nurse to perform home health visits, providing exceptional medical care to adult and geriatric clients in Fitchburg, Leominster, Harvard, Westford and surrounding towns. Responsibilities

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
