Job summary

BAYADA Home Health Care is seeking a part-time Licensed Practical Nurse to perform home health visits in Assisted Living and Independent Living communities across Millis, Norwood, Westwood and Dedham. You will provide compassionate medical care to geriatric patients in these senior living communities.

A current Massachusetts LPN license and at least one year of sub-acute/rehab experience are required. BAYADA offers a comprehensive benefits plan including paid holidays, vacation and sick leave,

Qualifications

• A current license as a Licensed Practical Nurse in Massachusetts.
• Minimum of one year of recent sub-acute/rehab experience as an LPN.

Responsibilities

• Making home visits to clients living in senior living communities
• Performing duties including administration of medication, wound care, treatments, and procedures.
• Monitoring clients’ conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and documenting doctors’ orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Document observations, interventions, and evaluations in EMRs on a touch pad tablet.

BAYADAHome Health has an immediate opening for a part-time Licensed Practical Nurse to perform home health visits in Assisted Living and Independent Living Communities, in Millis, Norwood, Westwood and Dedham. You will provide exceptional medical care to geriatric patients in these Senior Living Communities. Prior experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in senior living communities
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients’ conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors’ orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
• Minimum of one year of recent sub-acute/rehab experience as an LPN.
