Job summary

BAYADA Home Health Care is seeking a part-time Licensed Practical Nurse (LPN) to perform home health visits in Millis, Norwood, Westwood and Dedham, MA. You will provide geriatric care in senior living communities with a $2,500 sign-on bonus and a flexible schedule.

Qualified candidates must hold an active Massachusetts LPN license and have at least one year of sub-acute/rehab experience. Base pay ranges from $45.00 to $53.00 per hour, with full BAYADA benefits and support.

Qualifications

• A current license as a Licensed Practical Nurse in the state of Massachusetts.
• Minimum of one year of recent sub-acute/rehab experience as an LPN.

Responsibilities

BAYADA has an immediate opening for a part-time Licensed Practical Nurse to performhome health visits in Assisted Living and Independent Living Communities, inMillis, Norwood, Westwood and Dedham. You will provide exceptional medical care to geriatric patients in these Senior Living Communities. Prior experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in senior living communities
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
• Minimum of one year of recent sub-acute/rehab experience as an LPN.
