Job summary

BAYADA Home Health Care is seeking a full-time Licensed Practical Nurse to perform home health visits for adult and geriatric clients in Fitchburg, Leominster, Harvard, Westford and surrounding towns. Prior LPN experience is required. Call or text Susan at 339-235-4363 for immediate consideration.

Compensation includes base pay in the $45.00–$50.00 per hour range plus quarterly bonus opportunities and comprehensive benefits. BAYADA emphasizes quality patient care and work/life balance.

Qualifications

• Current MA LPN license.
• Prior clinical LPN experience required.

Responsibilities

• Make home visits to designated territories.
• Administer medications, perform wound care, treatments and procedures.
• Monitor client conditions and report changes to Clinical/Client Services Manager.
• Document and follow up on doctors’ orders.
• Perform client assessments as needed.
• Coordinate case management and care.
• Document observations, interventions, and evaluations in EMR on a tablet.

BAYADA Home Health has an immediate opening for a full-time Licensed Practical Nurse to perform home health visits, providing exceptional medical care to adult and geriatric clients in Fitchburg, Leominster, Harvard, Westford and surrounding towns. Prior clinical experience as a Licensed Practical Nurse is required.

Responsibilities

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
