Astrana Health, Inc. is seeking a SNF at Home Navigator in Johnston, RI to guide patients and families through transitions from hospital or SNF settings into SNF at Home care.

This field-based role reports to Population Health and partners with clinical teams, discharge planners, SNFs, and home care teams to ensure smooth enrollments and engaged, high-quality transitions. The ideal candidate is a Registered Nurse with an Associate degree, valid RN license, and experience in care coordination or

Qualifications

• RN license required.
• Associate's degree in Nursing required.
• Valid driver's license and reliable transportation.
• Experience in care coordination or discharge planning preferred.
• Familiarity with SNFs and post-acute care preferred.

Responsabilités

• Engage patients and families about SNF at Home options and enrollment.
• Coordinate transitions from hospitals or SNFs to home-based care.
• Support care plans and follow-up communications with providers and facilities.
