Job summary

Astrana Health, Inc. is seeking a RN Navigator to support patients and families transitioning to SNF at Home across Rhode Island. This role engages caregivers, hospitals, SNFs, and care teams to educate, coordinate and ensure a positive home-based care experience.

The RN Navigator collaborates with discharge planners and providers to facilitate enrollment, track progress, and maintain HIPAA-compliant documentation while traveling in-state.

Qualifications

• RN license required or active RN license.
• Experience in healthcare administration, care coordination, discharge planning, or patient navigation preferred.
• Experience working with patients and families during transitions of care preferred.

Responsibilities

• Engage patients and families to discuss SNF at Home options, eligibility, and enrollment.
• Educate on benefits, services, and logistics of home-based skilled nursing care.
• Coordinate outreach and transition activities with clinical teams and discharge planners.
• Conduct in-person field visits to hospitals/SNFs across Rhode Island and build relationships with care teams.
