Qualifications

• Strong background in home health and hospice services.
• Unrestricted RN license in practice state(s).
• Experience as a navigator or discharge planning role.

Responsibilities

• Collaborate with care teams to coordinate home-based discharges.
• Guide patients and families through post-acute care decisions.
• Identify and mitigate financial and clinical risks for patients.
• Participate in rounds as a patient advocate for continuity of care.
• Initiate early conversations about care destinations and home health options.
• Help prioritize patients who benefit most from home-based services.
