Job summary

Caretenders, a part of LHC Group and member of the Optum family, seeks a Speech Therapist for Home Health in a PRN capacity. You will assess speech, language, swallowing, and cognitive needs, then develop and monitor treatment plans to improve patient well-being and communication.

Responsibilities include collaboration with the health care team, documentation of progress, and educating patients and families. Requires state licensure, CPR, reliable transportation, and weekend availability.

Qualifications

• Licensed in Speech Therapy in the state of residence.
• Current CPR certification.
• Current driver’s license with vehicle or reliable transportation.
• Weekend availability.

Responsibilities

• Assess and evaluate patient care needs related to speech, language, swallowing, and cognitive impairments.
• Develop and implement a treatment plan and document progress.
• Work with the health care team to coordinate care and plan of care.
• Instruct patient, family and caregivers in home therapy activities.
• Observe, record and report patient responses to treatment.
