Qualifications

• Master’s degree in speech-language pathology.
• License to practice as an SLP in the state(s) where services are provided.
• Valid driver’s license and automobile insurance.
• CPR/BLS certification required.

Responsibilities

• Makes home visits to patients requiring speech-language pathology services and conducts ongoing interdisciplinary assessments.
• Performs initial and comprehensive assessments per agency policies.
• Evaluates language, swallowing, cognition, and memory functions as needed.
• Oversees treatment modalities and devices used in therapy.
• Completes clear, concise clinical documentation per home health CoPs and payer criteria.
