Interim HealthCare of SW Virginia is seeking a Home Health Speech-Language Pathologist - SLP to serve Roanoke, Salem and surrounding areas in patients' homes. This PRN role offers flexible scheduling and one-on-one therapy focused on communication, swallowing, and cognitive goals.

You will perform in-home assessments, develop treatment plans, document progress, and educate families while collaborating with the care team to support patient outcomes.

Qualifications

• Master’s Degree in Speech-Language Pathology required.
• Active, unrestricted SLP license in Virginia.
• Current CPR/AED/BLS/First Aid certification required.
• Minimum of 1 year of professional SLP experience (home health preferred).
• Reliable transportation and willingness to travel to patient homes.

Responsibilities

• Provide in-home speech therapy services for patients with communication, swallowing, or cognitive impairments.
• Evaluate patients’ speech and language abilities and perform periodic re-evaluations.
• Develop individualized treatment plans and therapeutic programs.
• Administer diagnostic and therapeutic techniques and exercises.
• Document patient progress and maintain accurate clinical records.
• Educate caregivers and family members on therapy goals, exercises, and safety practices.
• Collaborate with interdisciplinary care team members to coordinate patient care.
• Maintain effective communication with physicians, caregivers, and care team members.

Pay: $45-$100 per visit depending on visit type + mileage reimbursement
