Job summary

Compassus is seeking a Speech Language Pathologist to provide home health services under the direction of the attending physician, with active involvement from the family and care team.

You will perform initial and ongoing assessments (OASIS/Agency) of speech, language, cognitive, and swallowing disorders, develop and revise the Plan of Care with the physician, educate clients/families, and ensure quality and continuity of care with coordinated referrals.

Qualifications

• Bachelor's degree in Speech Language Pathology is required.
• Master's degree in Speech Language Pathology is preferred.
• Minimum of one (1) year of experience in a home health setting is required.
• Experience working with adult/geriatric patients is required.

Responsibilities

• Provides care utilizing infection control measures that protect both the staff and the patient.
• Demonstrates commitment, professional growth, and competency through in-services and required programs.
• Accepts clinical assignments consistent with education and competence.
• Monitors assigned cases to ensure compliance with third-party payors.
• Communicates effectively with patient/family, team, physician, and other health care professionals to coordinate services.
• Provides speech therapy services and treatments requiring specialized skills.
• Educates client/family regarding the disease process.
• Performs initial and ongoing comprehensive assessment of speech, language, cognitive, and swallowing disorders with appropriate tests and measures and maintains records including goal setting.
• Develops/implements Plan of Care in consultation with physician and care team to ensure quality and continuity of care.
• Performs other duties as assigned.
