Job summary

Compassus is hiring a Home Health Speech Language Pathologist to provide speech and language therapy to adults and geriatric patients in a home health setting. You will develop plans of care, perform assessments, and coordinate with physicians and families to support safe, effective communication and swallowing care.

You will educate staff and families, document care accurately, participate in team meetings, and help ensure quality care within the interdisciplinary plan of care.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting.
• Experience working with adult and geriatric patients is required.

Responsibilities

• Develop speech therapy plans of care after consultation with referring physician.
• Visit patients for assessment and evaluation; complete OASIS assessments.
• Educate team members, patients, and families.
• Document care and submit paperwork timely to update medical records.
• Provide speech/language pathology services per interdisciplinary plan of care.
