Job summary

Excellent Home Care NY is seeking a Speech-Language Pathologist to deliver in-home rehabilitative services in the New York area. The role involves evaluating patients, coordinating with nurses and family, and documenting progress and discharge plans.

The ideal candidate holds a Master’s in Speech-Language Pathology, NY licensure, and ASHA certification, with at least 1 year of healthcare experience and preference for home care experience.

Qualifications

• Master's degree required in Speech-Language Pathology.
• Minimum of 1 year of SLP experience in a health care setting.
• Current NYS licensure as an SLP and ASHA certification.

Responsibilities

• Plan and provide in-home Speech-Language Rehabilitation services.
• Collaborate with health team to set rehab goals and activities.
• Document treatments, progress notes, discharge summaries, and plans of care.
• Communicate with nurses and family; adjust plan as needed.
• Re-evaluate patients to assess progress and update care plans.
