• Minimum six months experience as a speech therapist or speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function.
• Observe, record and report changes in patient condition to supervisor or physician.
• Provide instruction and training in alternative communication systems when appropriate.
• Maintain and submit documentation as required by CenterWell Home Health.
