Select Medical in Jeffersonville, IN is seeking a Home Health Speech Language Pathologist (SLP) to provide in-home therapy services, evaluate patient status, and coordinate discharge planning. The role supports patients in southern Indiana through the home health program and collaborates with KORT Home Health partners.

The SLP will use evidence-based treatment plans, monitor progress, and educate patients and families while meeting licensing, CPR, and ASHA CCC-SLP requirements.

Qualifications

• Master's degree in speech pathology required.
• ASHA CCC-SLP credential required.
• Minimum two years speech pathology experience.
• Must have automobile liability insurance and a valid, unrestricted driver's license.
• CPR certification or completion of CPR training.

Responsibilities

• Provide skilled therapy intervention and treatment to patients in the home setting. This includes using treatment plans and assessment skills.
• Careful monitoring of patient status, progress toward goals, and physicians orders compliance are essential.
• Speech language pathologists teach patients and caregivers and prepare the patient/family for discharge as appropriate.
