Job summary

Compassus is seeking a Home Health Speech Language Pathologist to provide evaluation and treatment for patients with speech and language disorders in their homes in Indiana, including Kokomo. The role operates under physician supervision with family involvement as needed.

Responsibilities include performing visits for assessment, developing plans of care, completing OASIS, and educating patients and families. Strong communication and licensure as a Speech Language Pathologist are required.

Qualifications

• Mathematical skills to perform calculations for therapy planning and documentation.
• Strong written and verbal communication for patient and team interaction.
• Ability to understand, read, write, and speak English and to articulate hospice philosophy.

Responsibilities

• Develops speech therapy plans of care after physician consultation.
• Visits patients for assessment and evaluation and completes OASIS assessments.
• Educates patients, families, and team members.
• Documents care and maintains medical records accurately and timely.
• Participates in interdisciplinary team meetings and quality improvement programs.
• Ensures care aligns with patient rights and safety standards.
