Job summary

HealthCare Therapy Service is seeking a licensed Speech Language Pathologist (SLP) to provide home health therapy in Columbus, Indiana.

The SLP will assess patients, develop treatment plans, monitor progress, educate families, and collaborate with care teams while maintaining compliant documentation. Licensure in Indiana or eligibility to earn it, and CCC or CF supervision, are required.

Qualifications

• Active Indiana SLP license or qualifications to obtain one.
• Certificate of Clinical Competence (CCC) required or be supervised as a Clinical Fellow.
• Legal eligibility to work in the United States.

Responsibilities

• Assess patients' functional abilities and develop personalized treatment plans to improve independence and quality of life.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate patients and their families on strategies to support ongoing health and wellness.
• Collaborate with healthcare teams, caregivers, and facility staff to ensure comprehensive patient care.
• Maintain accurate and timely documentation in compliance with regulatory standards.

Requirements

Applicants must have an active Indiana Speech Language Pathologist license or must have the necessary qualifications to earn Indiana license, including completion of a nationally accredited program and subsequent passing of national licensure exam.
