Job summary

Qualifications

• Master’s degree in Speech-Language Pathology required.
• 1–2 years of clinical experience required.
• Licenses/certifications required: SLP license (MD/VA/DC as applicable), driver's license, CPR.

Responsibilities

• Contributes to goals and adheres to policies; completes mandatory training.
• Arranges specialized assessments and plans discharge including referrals to community resources.
• Communicates with supervisor regarding rehab services and patient care.
• Maintains relationships with other departments and participates in quality and service improvement teams.
• Maintains communication with patient’s physician and care team regarding care and status.
• Assesses patient environment and recommends adaptive equipment or home modifications.
• Provides initial and periodic assessments and develops/modifies speech-language treatment plans.
• Documents medical records per agency standards for reimbursement.
• Educates patients and caregivers about the injury/disease process and care after discharge.
• Provides direct SLP services and supervises home health aides as needed.
• Participates in meetings and represents the department in community outreach.
