Job summary

Assured Home Health, part of the Optum/LHC Group family, seeks a Social Worker to assess social and emotional needs to support patient health. You will collaborate with physicians and the interdisciplinary team to craft plans of care and connect patients with community resources.

Requirements include a Master’s in Social Work, a valid driver’s license, and the ability to arrange transportation. CPR or future completion within 90 days is preferred; licensure as an SW in the state is also

Qualifications

• Master's Degree from a school of Social Work accredited by the Council on Social Work Education.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Licensed Social Worker in the state of residence (preferred).
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Assists patient, family, physician and care team to understand social and emotional factors affecting health.
• Contributes to a comprehensive Plan of Care and coordinates community resources.
• Instructs care team on resources available to support patients.
