Job summary

Assured Home Health, part of LHC Group and within the Optum family, seeks a Social Worker to provide medical social services under physician direction to address social and emotional factors affecting patient health.

You will collaborate with the interdisciplinary team to develop a comprehensive plan of care and educate on community resources, ensuring patient-centered care. A social work credential and driving flexibility support field-based responsibilities.

Qualifications

• Master's degree from a school of Social Work accredited by the Council on Social Work Education (CSWE).
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Licensed Social Worker in the state of residence (preferred).
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Assist patients, families, physicians and care team to understand social and emotional factors related to health.
• Contribute to the development of a comprehensive, integrated Plan of Care daily.
• Instruct health care team on community resources available to assist patients as needed.
