Humana Inc. through CenterWell Home Health is seeking a Medical Social Worker to assess and plan care addressing patients’ social and emotional needs in the home.

You will work with the interdisciplinary team to support patient recovery and coordinate resources, while supervising the Social Worker Assistant when needed. The position requires at least one year of health care social work experience, state licensure where required, a valid driver’s license, CPR, and ability to travel locally.

Formación

• Minimum of one year of experience as a social worker in health care, home health or hospice.
• Social Worker licensure in the state of practice, if required by state law or regulation.
• A valid driver's license, auto insurance, and reliable transportation.
• CPR certification is required.
• Experience with discharge planning and obtaining community resources.
• Knowledge of medications and their correct administration.
• Ability to organize tasks, develop action plans and manage stress.
• Flexible hours and ability to travel locally.
• Maintain current licensure and continuing education requirements.
• Must read, write and speak fluent English.

Responsabilidades

• Assess the patient's social and emotional state related to illness or injury and care needs.
• Assess relationships of medical/nursing needs, finances and community resources.
• Facilitate access to community resources to support recovery.
• Communicate with patients, families and health team members.
• Provide ongoing assessment of patient and family needs and responses.
• Participate in the interdisciplinary team for care coordination.
• Supervise Social Worker Assistant when involved in care planning.
• Maintain and submit required documentation and notes.
• Use skills to make an impact.
