Assured Home Health is seeking a Social Worker to provide medical social services under physician direction and an interdisciplinary group. The role focuses on understanding social and emotional factors affecting patient health and developing coping mechanisms, with a goal of coordinating a comprehensive Plan of Care.

The position emphasizes collaboration with the health care team, community resource guidance, and ongoing professional development within a supportive environment.

Qualifications

• Master's Degree from a CSWE-accredited school of Social Work.
• Current Driver's License and access to a dependable vehicle or public transportation.
• Preferred: 1+ years of social work experience in a health care setting.
• Current CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Assist patients, families, physician and care team to understand social factors related to health status.
• Contribute to development of a comprehensive Plan of Care on a daily basis.
• Instruct team members on community resources available to assist patients.
