Job summary

PurposeCare is seeking a dedicated Home Health Registered Nurse (RN) for a full-time role. You will manage a patient caseload in the home setting, delivering high-quality care with autonomy and collaboration with physicians and the interdisciplinary team.

The ideal candidate holds an active RN license, has at least one year of Home Health experience, and is skilled in OASIS documentation, assessment, and patient education. A valid driver’s license and reliable transportation are required.

Qualifications

• Active Registered Nurse (RN) license.
• Minimum of one year of recent Home Health experience.
• Strong knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision-making skills.
• Outstanding communication and relationship-building abilities.
• Valid driver's license, reliable transportation, and auto insurance.
• Passion for delivering patient-centered care in the home setting.

Responsibilities

• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans that support patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete timely documentation and OASIS assessments.
• Monitor patient progress and adjust care plans as clinical needs change.
• Supervise Home Health Aides and collaborate with LPNs as appropriate.
• Participate in case conferences and interdisciplinary care planning.
• Help improve patient outcomes while maintaining Medicare participation and agency standards.

What You'll Do
