Zusammenfassung

CenterWell Home Health seeks a Speech Language Pathologist to evaluate and treat patients in homes or facilities, developing plans of treatment and care with the care team. You will assess speech, voice, language and swallowing disorders and provide counseling to patients and families.

The role requires ASHA CCC-SLP certification, at least six months of SLP experience, current licensure, CPR, a valid driver’s license, and auto insurance.

Qualifikationen

• CCC-SLP/ASHA certification required.
• 6+ months experience as SLP preferred.
• Current and unrestricted license to practice.
• CPR certification required.
• Valid driver’s license with auto insurance and reliable transportation.

Aufgaben

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders.
• Administer and interpret diagnostic tests and evaluation procedures.
• Record changes in patient condition and communicate with supervisor/physician.
• Provide instruction to patients and families in communication strategies.
