CenterWell Home Health is hiring a Speech Language Pathologist to deliver in-home or facility-based services in Georgia. You will evaluate and treat speech, voice and swallowing disorders, develop care plans, and support patients to regain independence and quality of life.

The role requires ASHA CCC or eligibility, 6+ months of experience, a valid Georgia license, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Meet the education and experience requirements for Certification in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted state license and CPR certification.
• Valid driver's license and reliable transportation.

Responsabilités

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and related therapies.
• Record and report changes to supervisor or physician; document progress and care plans.
• Counsel patients and families; participate in care coordination and discharge planning.
