Job summary

Adaptive Home Health, LLC is seeking a Speech-Language Pathologist (SLP) to join our field-based home health team in Texas. You will conduct in-home evaluations, deliver skilled visits, and manage SLP plans to drive functional communication and swallowing safety, with caregiver coaching to ensure progress between visits.

The role emphasizes independent practice, strong documentation, and coordination with a multidisciplinary team in a productive, patient-centered environment.

Qualifications

• Active Texas SLP license (in good standing).
• Strong clinical reasoning and ability to operate independently in the field.
• Comfort identifying red flags and escalating changes in condition to RN/MD.
• Strong caregiver education skills (teach-back, carryover planning).
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.

Responsibilities

• Own outcomes for the patient’s SLP plan, progression and functional outcomes.
• Conduct comprehensive in-home SLP evaluations and deliver skilled visits.
• Assess and treat aphasia, cognitive-communication issues, and dysphagia (as ordered).
• Build and execute a clear plan of care with measurable goals.
• Drive multidisciplinary coordination and keep the care team aligned.
• Coach patients and caregivers with practical strategies and routines.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
