Job summary

Adaptive Home Health in Texas is seeking a Speech-Language Pathologist (SLP) to deliver in-home care across a field-based territory. This full-time role offers Monday–Friday scheduling with occasional weekends and compensation based on productivity points at $75/point.

You will conduct comprehensive in-home evaluations, provide skilled SLP visits, address aphasia, cognitive-communication issues, and dysphagia, and work with the care team to establish measurable goals.

Qualifications

• Active Texas SLP license (in good standing).
• Strong clinical reasoning and ability to operate independently in the field.
• Comfort identifying red flags and escalating changes in condition to RN/MD.
• Strong caregiver education and teach-back skills.

Responsibilities

• Own outcomes: manage the patient’s SLP plan, progression, and functional outcomes.
• Perform comprehensive in-home SLP evaluations and visits.
• Assess and treat aphasia and other communication disorders, cognitive-communication impairment, and dysphagia as ordered.
• Build and execute a clear plan of care with measurable, functional goals.
• Coordinate with the care team, communicating changes in condition and aligning orders.
• Coach patients and caregivers with practical strategies and routines.
• Document promptly in Homecare Homebase (HCHB).
