Job summary

VitalCaring is seeking a Home Health Speech-Language Pathologist (SLP) to evaluate and treat patients in their homes. You will lead assessments, develop plans, and deliver evidence-based therapy focused on communication, cognition, and swallowing.

This role emphasizes autonomy with clinical leadership support and direct, one-on-one patient care. The ideal candidate holds a Master’s or Doctoral degree in Speech-Language Pathology, an active state license, CCC‑SLP preferred, CPR certified, and a

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology is required.
• Current SLP license in state of practice is required.
• CCC‑SLP certification preferred.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead evaluation and treatment of patients with speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized plans of care.
• Provide evidence-based therapy and assistive strategies.
• Assess home environment and recommend adaptive equipment.
• Educate patients and caregivers on exercises and safety techniques.
• Document care accurately in the EMR and coordinate with care team.
