Job summary

Addus HomeCare in Nashville, TN is seeking a PRN Speech Language Pathologist to deliver one-on-one home health care. You will evaluate, treat, and create plans of care to help patients recover, manage chronic conditions, and maintain independence in their own homes.

The role requires ASHA certification, state licensure, CPR, and reliable transportation for home visits. You may participate in an on-call schedule that includes evenings and weekends as patient needs dictate.

Qualifications

• ASHA certification in speech-language pathology is required.
• Licensed as a speech-language pathologist in the practicing state.
• At least 1 year of acute care or equivalent experience.
• Knowledge of principles and practices of speech therapy.
• Current CPR certification is required.
• Ability to make home visits and provide care in the home environment.
• Must have reliable transportation, valid driver's license, and appropriate auto insurance.

Responsibilities

• Conduct evaluations and assess home environment as appropriate.
• Identify functional speech defects and establish a plan of care.
• Administer diagnostic speech and language evaluations and informal assessments.
• Recommend communication devices or aids as indicated.
• Prepare reports of evaluations and recommendations.
• Advise, consult and train clients, family, and staff.
• Provide direct speech therapy and document client progress to the provider.
