Addus HomeCare, Inc. in Davidson County, TN, is hiring a PRN Speech Language Pathologist to deliver one‑on‑one patient care in home health. Flexible weekday visits with rotating on‑call including evenings and weekends; daily hours vary by patient needs, travel, and documentation.

You will conduct evaluations, plan care, and provide speech therapy services in patients' homes, coordinating with providers and families to maintain independence.

Qualifications

• Certification of clinical competence in speech from the American Speech-Language-Hearing Association (ASHA).
• Licensed as a speech therapist in the practicing state.
• CPR certification current and valid.
• Ability to make home visits and provide nursing care in the home environment.

Responsibilities

• Conduct evaluations and assess home environment and equipment needs relevant to speech/language function.
• Identify functional speech defects and plan care to improve function.
• Administer diagnostic evaluations and informal audiology assessments per care plan.
• Recommend communication devices and aids as indicated.
• Prepare reports of diagnostic evaluations with recommendations.
• Advise and teach procedures to clients, families, and staff.
• Provide direct speech services and report client responses to the provider.

What You’ll Do:

• Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
• Identifies functional speech defects and establishes a plan of care to improve patient’s function.
• Administer diagnostic speech and language evaluations and some basic, informal audio logical assessment in accordance with the plan of care
• Recommends communication devices/aids as indicated.
• Prepare a report of each client’s diagnostic evaluation and make recommendations
• Advise, consult and teach appropriate procedures and supportive activities to clients, family members, and other agency staff
• Provide direct speech service to clients under a provider’s plan of care. Records and reports to the provider the client’s response to therapy

Qualifications:

• Possess certification of clinical competence in speech from the American Speech Language Hearing Association
• Licensed as a speech therapist in the state in which practicing
• At least one (1) year of acute care setting or equivalent experience
• Possess knowledge of principles and practices of speech therapy
• Complies and maintains current CPR certification.
• Ability to make home visits and provide nursing care in the home environment
• Must have reliable transportation, current driver’s license, and appropriate automobile insurance
