Personal Touch Home Care in Brooklyn, NY is expanding its Special Needs Program and is seeking compassionate Speech Language Pathologists who are passionate about delivering high-quality care in the home setting. This 1099, contracted, fee-for-service position pays $100 per visit and requires a valid NY license.

The role involves evaluating and treating individuals with speech and language disorders; programs cover pediatric, adult, and geriatric populations in patients' homes.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Demonstrates good verbal and written communication and organization skills.
• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.

Responsibilities

• Conduct comprehensive assessments of speech and language abilities.
• Develop and implement individualized treatment plans tailored to each patient's needs.
• Provide therapy to individuals of all ages, addressing issues such as articulation, fluency, voice, and language.
• Collaborate with other healthcare professionals to provide holistic care.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate patients and their families about communication strategies and home exercises.
• Stay current with best practices and advancements in speech therapy.
