Job summary

Personal Touch Home Care is seeking a Speech Language Pathologist (SLP) for home-based care in East New York and Canarsie, Brooklyn. This full-time role supports pediatric and adult caseloads with 30–60 minute sessions, in patients' homes across targeted borough neighborhoods.

Qualifications

• Active New York State SLP license required.
• Graduate of an accredited Speech Language Pathology program.
• Strong communication and organizational skills.
• Home care experience preferred, but not required.
• Experience with pediatric, adult, special needs, or geriatric populations is a plus.

Responsibilities

• Conduct comprehensive assessments of speech and language abilities.
• Develop and implement individualized treatment plans for each patient.
• Provide therapy to individuals of all ages within home-based care settings.
• Collaborate with other healthcare professionals to ensure holistic care.
• Monitor and document patient progress, adjusting plans as needed.
• Educate patients and families about home exercises and strategies.
• Stay current with best practices in speech therapy.
