Compassus is seeking a Home Health Speech-Language Pathologist (PPV) to deliver comprehensive SLP services in the home health setting. The role emphasizes patient-centered care, collaboration with physicians and families, and maintaining accurate documentation in the EMR.

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one (1) year of experience in a home health setting required.

Responsabilidades

• Develop speech therapy plans of care after physician consultation.
• Visit patients for assessment/evaluation and complete OASIS assessments.
• Provide education to team members, patients, and families.
• Document care and maintain medical records accurately.
• Participate in quality improvement programs.
• Promote growth and profitability through responsible use of resources.
• Coordinate with physicians and staff regarding patient condition and records.
• Deliver speech-language pathology services per the care plan, focusing on communication and swallowing exercises.
