Job summary

Compassus is seeking a Home Health Speech Language Pathologist - PPV PRN to assess and treat speech, language, cognitive, and swallowing disorders in home health settings. You will work with patients and families, complying with regulatory standards and documenting care plans.

With a Bachelor’s in Speech Language Pathology and a licensed practice, you will participate in ongoing assessments, collaborate with physicians, and contribute to high-quality patient outcomes in a compassionate home-care

Qualifications

Bachelor’s degree in Speech Language Pathology is required.
Master’s degree in Speech Language Pathology is preferred.
Minimum of one year of experience in a home health setting is required.

Responsibilities

Provides care utilizing infection control measures that protect staff and patients.
Accepts clinical assignments consistent with education and competence.
Monitors assigned cases to ensure third-party payor compliance.
Communicates with patient/family and care team to coordinate services.
Documents Plan of Care and evaluates outcomes.
Supervises Home Health Aide/UP for quality and continuity of service.
Performs comprehensive assessment of speech, language, cognitive, and swallowing disorders and maintains records.
Updates Plan of Care in consultation with physician and care team members.
