Job summary

Qualifications

• High school diploma or equivalent.
• Three years total of medical office experience.
• One year of scheduling experience.
• One year of Home Health Agency Operations experience.
• Experience with HomeCare HomeBase (HCHB).

Responsibilities

• Provide accurate schedules for all patients from referral through admission and discharge.
• Participate in agency back office daily operations as assigned by BOM.
• Demonstrate flexibility and address changes with schedule adjustments.
• Maintain visit productivity for various partner levels within the agency.
• Update the daily Home Health RADAR staffing availability report.
• Interact with clinicians, intake, nurses, and other staff to meet scheduling needs.
• Be flexible to work beyond normal hours on weekends and holidays as needed.
• Ensure confidentiality and PHI is protected at all times.
• Document coordination notes and scheduling processes in HCHB.
