Job summary

Qualifications

• Completion of an accredited nursing program.
• RN license in the state of practice.
• BLS for Healthcare Providers certification within hire date.
• Typically 1 year of nursing experience.

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts patient assessments and updates the plan of care.
• Follows care plan and evaluates outcomes.
• Communicates with physicians to update treatment plans.
• Documents patient records per guidelines.
• Discharge planning with RN Case Manager and utilizes community resources.
