Job summary

Almost Family, part of LHC Group and a member of the UnitedHealth Group family, is seeking a Registered Nurse to provide and direct nursing care to patients in their homes. You will coordinate total plans of care with other health professionals, ensuring continuity of care and compliance with laws and policies.

Responsibilities include high-quality clinical services, coordinating care from admission to discharge, and developing individualized care plans.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• Current CPR certification.
• Ability to function in diverse home environments.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meets patient needs and obtain physician approval for revisions.
