Almost Family, part of LHC Group and UnitedHealth Group family, is hiring a Per Diem Registered Nurse to provide and direct nursing care in patients' homes. You will collaborate with physicians and a care team to ensure continuity of care and optimal patient outcomes in a flexible home-health setting.

The role emphasizes independent work, adherence to regulations, and delivering compassionate, patient-centered services across diverse communities.

Formación

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• Ability to work independently with good communication and organizational skills.

Responsabilidades

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meet patient needs and obtain physician approval.
• Function effectively in diverse home settings and with diverse patient needs.
