Job summary

UnitedHealth Group is seeking a Registered Nurse in home health to provide and direct nursing care in patients' homes, coordinating the total plan of care with physicians and the health care team.

The role includes supervision of home health aides, timely documentation, and ensuring quality care, with on-call/weekend rotation as needed. CPR certification preferred or within 90 days of hire.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver's license and access to reliable transportation.
• CPR certification preferred or within 90 days.
• Ability to work independently.
• Strong communication and organizational skills.

Responsibilities

• Initiates and coordinates the plan of care with the physician and health care team.
• Supervises home health aides and licensed nurses per regulations.
• Provides supervisory visits and ensures compliance with policies.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents patient visits per policy and coordinates with physicians, patients and families.
• Takes part in on-call and weekend rotations and supports care continuity.
