Job summary

Maxim Healthcare is seeking Registered Nurses (RNs) to join our Transition Team to support patients moving home as permanent staff are recruited. The team provides in-home care during a provisional 1–90 day period, with office staff recruiting permanent caregivers afterward.

Responsibilities include nursing assessments, plans of care, medication administration, and patient education. Flexible full- and part-time roles are available with competitive hourly pay.

Qualifications

• Current RN license in the state of practice.
• Current Health Certificate (per contract or state regulation).
• Current PPD or Chest X‑Ray
• Current BLS certification
• Ability to travel 45 minutes to an hour
• Ability to work with pediatrics and adults
• Ability to care for (training available) patients with varied acuity
• One year of RN experience preferred

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and update plans of care as directed; request provider revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
• Respond to changes in patient condition and report to physicians or supervisors as appropriate
• Administer prescribed medications and treatments and document patient response
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning
