Job summary

Maxim Healthcare Services, Inc. is seeking Registered Nurses to join the Transition Team, providing care during patient transitions home and supporting the recruitment of permanent caregivers.

Positions offer full-time and part-time options with competitive hourly pay and weekly paychecks. The Transition Team delivers nursing care across all authorized hours in the home during a provisional 1–90 day period, with training and shadowing available before assuming independent care.

Qualifications

• Current RN license in the state of practice.
• Current Health Certificate (per contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS certification.
• One year of RN experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and update plans of care as directed; request provider revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
• Respond to changes in patient condition and report to physicians or supervisors as appropriate
• Administer prescribed medications and treatments and document patient response
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning
